Coordinates the clinical pharmacy, including developing, monitoring, evaluating and coordinating clinical pharmacy services that promote desirable patient outcomes. The incumbent will develop and implement policies, procedures, process guidelines, and protocols to ensure the provision of safe, effective and economical pharmacotherapy for all patients in the hospital, ensuring compliance with clinical practice standards.
PRIMARY RESPONSIBILITIES
Collaborates with medical staff, nursing, administration and pharmacy director to maintain productive and effective relationships to ensure the sa f e and effective delivery of pharmacy services across the continuum of care. Proficient in conflict resolution, group dynamics and facilitation of significant projects.
Participates in, coordinates or leads committees and/or functions as appropriate and/or required by the organization.
Development and implementation of quality improvement initiatives, fiscal controls, facilitates of human and capital resources, formulary management, and risk management functions for the clinical pharmacy program.
Remains up to date on advances and changes in pharmacy related subjects through continuing education to possess most up-to-date knowledge of meet clients' needs.
Provides input and recommendations to revise clinical guidelines and or procurement of educational materials to assure best practices.
Maintains competency and skills and is able to perform duties of the staff pharmacist when needed.
Coordinates of clinical pharmacy staff who provide patient-focused pharmaceutical care.
Works with pharmacy buyer, medical staff, and director to evaluate and select supplies that optimize product selection and pharmacy purchasing.
Develops and presents continuing education programs for pharmacists, physicians, nurses, students and others to assure providers have current knowledge of therapies and risks.
Procures or creates educational materials for providers and for patient education.
Works in a constant state of alertness and safe manner.
Performs other duties as assigned
EDUCATION
Pharmacy degree ( Pharm.D ; Bachelor of Pharmacy) from an ACPE accredited school of pharmacy.
EXPERIENCE
Two years experience
PHYSICAL REQUIREMENTS
Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.
Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
Frequent keyboard use/data entry.
Occasional bending, stooping, kneeling, squatting, twisting and gripping.
Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
Rare climbing.
