Job Summary and Scope

Consult with and educate physicians, other health professionals, patients and their families about drug usage, drug dosage, and drug substitution. Focus on cost effective and safe usage of drugs in inpatient, outpatient, and ambulatory settings and improving the quality of care given to patients.

Compound and dispense medications and pharmaceutical supplies on a written order from a licensed independent practitioner (LIP). Participate in continuous quality improvement program at University of New Mexico-Sandoval Regional Medical Center (UNM-SRMC). Oversee pharmacy technicians and interns in the performance of their duties. Responsible for providing the best customer service to both internal and external customers. Ensure adherence to State and Federal regulations as well as Hospital and Departmental policies and procedures. Patient care assignments may include neonate, pediatric, adolescent, adult and geriatric age groups.

Essential Functions:

POLICIES AND PROCEDURES - Maintain established departmental policies and procedures, objectives, and quality assurance programs

Enhance professional growth and development through participation in educational programs, reading current literature, attending in-services, meetings, and workshops.

Maintain professional environment in communications, duties, tasks, interactions, and customer service

Weigh, measure and mix ingredients; compound and prepare medication emulsions, liquids, powders, and ointments using graduates, balance, mortars, pestles, and funnels

Fill prescriptions or orders from bulk stock; fill bottles, capsules and other package forms with measured amounts of medication according to prescription; type prescription or order information for identification and use of medication; attach label to container following dispensing format

Ensure timely and accurate filling and delivery of physician's prescriptions or orders

Prepare and dispense doses of medications as required to meet patient needs

Mix and dispense additives for IV, IM, and irrigating solutions

Review non-formulary drug requests and recommend formulary alternatives when possible

Enter physician prescriptions and orders into pharmacy computer system; review physician prescription or orders for completeness and appropriateness; perform prospective review of patient profile, check for drug-drug, drug-food, drug-disease, drug supplement interactions, patient allergies, appropriate dose, admixture incompatibilities, drug dose, route schedule, etc

Collaborate with other healthcare professionals in the selection and individualization of drug therapy for patients

Perform parenteral nutrition monitoring, Renal drug dosing, IV to PO conversion, Anticoagulation therapy monitoring, antibiotic selection, Pharmacokinetic monitoring, and other monitoring duties as assigned

Confer with medical personnel to correct problems with physician prescription or orders; recommend changes in therapy with goal of enhancing patient care, increase patient safety and decrease costs; encourage adherence to formulary , IV to PO recommendations, clinical practice guidelines, and antibiotic selections

Provide concise, complete, applicable, and timely responses to drug information requests from healthcare professionals and patients

Participate in medication usage evaluations (MUE) and preparation of clinical practice guidelines

Responds to Code Blue, Rapid Response, and emergency situations.

Responds to on-demand or urgent requests for medications not stocked in the automated dispensing cabinets.

Perform pharmacy audits on a regular basis as required by department policy

Document patient information and recommendations regarding drug therapy regimens in patient charts when appropriate. Document clinical interventions in pharmacy departments clinical documentation software

Attend meetings and represent department or Hospitals within Hospitals related committees or the community, as assigned

Participate in the development of clinical practice guidelines and implementation of clinical order sets

Provides patient education and medication counseling.

Follow up on problems identified by pharmacy staff in their prescription or order coordination process; recommend process changes as needed

Develop, execute, and participate in quality improvement programs that enable the department to continue to improve pharmaceutical care to the patients

Organize and guide the daily operational duties of the support personnel and co-workers to provide best possible workflow; perform Charge Pharmacist duties when assigned

Educates hospital and medical staff both individual and collectively; train and educate residents, students, and technicians

Act as a liaison to nursing units and clinics

Facilitate interdepartmental communication and problem identification and resolution; provide communication between work areas and shifts to provide continuity of patient care

Participate in a program of self-development that will allow the pharmacist to develop a high level of clinical or technical expertise specific to the particular area of practice

Follow patient safety-related policies, procedures and protocols

Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes

Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk

Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner

Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand or may not seem right

Actively identify, mitigate, prevent, and report potential and actual drug-related problems or adverse drug events. Recommend rational therapeutic alternatives as needed to prevent future problems.

Collects data and performs inspections for quality assurance purposes. Maintains all logs and necessary paperwork associated with regulatory and quality assurance requirements. Actively participates in LEAN processes and quality improvement initiatives.

Perform other miscellaneous job-related duties as assigned by the Director of Pharmacy

Required Qualifications

Knowledge of pharmaceutical dosing forms, pharmaceutical and medical terminology, brand and generic names of medications

Knowledge of anatomy and physiology. Knowledge of pharmacology, clinical therapeutics, and pharmacokinetics are essential

Ability to integrate clinical services, distributive functions, and education

Knowledge of sterile compounding techniques

Knowledge in weighing, measuring, and mixing medication ingredients

Knowledge of inventory control and basic drug storage processes.

Knowledge in State and Federal regulation

Ability to maintain records including, but not limited to, narcotic inventories. and other controlled substance records

Knowledge of principles and processes for providing customer and personal services

Knowledge of arithmetic, algebra, statistics, and their applications. Knowledge of pharmaceutical calculations.

Knowledge of electronic equipment, computer hardware and software, including applications

Ability to be detailed oriented and consistently follow process

Critical thinking skills, judgment and decision making abilities

Ability to arrange things or actions in a certain order or pattern according to a specific rule or set of rules

Education/Experience

Education:

BS in Pharmacy or PharmD

Licenses/Certifications

Current State of New Mexico Pharmacist License without limitations or restrictions

BLS certification within 90 days of hire.

ACLS certification within 180 days of hire

PALS certification within 180 days of hire

Work Experience:

Will consider a new graduate.

Preferred Educational/Experience Requirements:

PharmD degree

Residency in an acute care setting

Pharmacy experience in a community or hospital pharmacy as a licensed pharmacist

Board Certification in Pharmacotherapy

Supervision

Will report to Director of Pharmacy

Conditions of Employment

Must pass a pre-employment criminal background check, reference checks and a post offer drug screen.

Must be employment eligible as verified by the U.S. Dept. of Health and Human Services Office of Inspector General (OIG) and the Government Services Administration (GSA).

Tuberculin Skin Test required annually

Working Conditions

Typical office and/or patient care, acute care hospital environment.

Must be able to travel locally between facilities and within the surrounding community.
